Skilled Nursing

The Skilled Nurse is an RN or LVN/LPN with training and experience in providing care in the home. The nurse

communicates frequently with your physician to update your plan of care. Services may include — but is not limited to:

  • Evaluation of Patient Needs,

  • Coordination of Patient Care,

  • Performance of Skilled Nursing Procedures,  

  • Disease Education of Diseases and Process,

  • Self-Care Techniques and Prevention Strategies

Home Health Aid

Home Health Aide includes assistance of daily living activities. Typical duties include — but is not limited to:

  • Bathing

  • Dressing

  • Grooming

  • Changing Bed Linen

  • Serving Meals & Snacks

These services are delivered under the supervision of a registered nurse or licensed therapist. Our aides are experienced in carings for those in need. 

Medical Social Worker

Medical Social Workers provide short-term services that includes — but is not limited to:

  • Counseling

  • Finances and Long-Range Planning

  • Community Referral and Coordination

  • Resources/Assistance with Living Arrangements

Supplies/Therapy/Equipment

Medical Supplies, Therapy Services, or Durable Medical Equipment (DME) may be required to carry out your plan of care. All medically necessary therapy services or medical supplies must be coordinated with the home health agency whole you are receiving home health services covered by a health plan. 

Durable Medical Equipment (DME)

Durable Medical Equipment (DME) includes — but is not limited to:

  • Canes

  • Walkers

  • Wheelchairs

  • Hospital Beds

​​

This equipment is covered separately and may be supplied by the home health agency or outside certified supplier of your choice.​
